(THC) content; industrial hemp is an agricultural crop for seed, fiber and hemp-derived CBD Oil. CBD oils derived from hemp typically contain only trace amounts of psychoactive THC (less than .3%). Historical Use of CBD (Cannabidial Oil) CBD oil found in the plant part (hemp) of the Cannabis Sativa plant has an effect on the endo-cannabinoid system in the brain, central and peripheral nervous system, fat, muscle tissue, and immune cells in humans. CBD interacts primarily with CB2 receptors with indirect actions on CB1 receptors and is thought to inhibit the release of glutamate, an excitatory neuro-transmitter and adenosine. 4 This is important because the inhibitory actions on the CB receptor play a role in maintaining normal brain activity and protecting the brain against seizures. 5,6 The CBD oil in this preliminary study was adminis-tered orally as gel-caps. There is some evidence that this dosage route can be inactivated somewhat by the stom-ach and liver before it can reach therapeutic levels in the patient's blood stream. Given the drawbacks of current methods of administration, oral dosage is the most compatible for dental phobic patients. In the future, improved formulations of CBD oil could improve the bioavailability of the medication. include a vitamin capsule in the same size and color as the CBD oil gel-cap to be used in a double blind study to eliminate the "placebo effect" of the medication. Neither the investigator nor the patient will have knowledge of the medication used in the study. The vial containing the medication will have a numerical code so that only the investigator at the company will know which code goes with which medication. The Endocannabinoid System (ECS) In order to gain some understanding of how CBD oil helps the apprehensive or dental phobic patient, the mechanism of the ECS system has to be better explained. In the simplest terms, the ECS consists of compounds known as endocannabinoids as well as the receptors they bind with. These receptors and endo-cannabinoids are found in virtually every main organ in the human body and play a major role in sleep, mood, memory and even pain sensation. 7 This ECS system was discovered by Dr. Raphael Mechoulam, an organic chemist and professor of medicinal chemistry, at the Hebrew University of Jerusalem in Israel in 1992. 8 As the pathophysiologic role of the ECS system is further elucidated, much more accurate targeting for clinically successful treat-ments will be obtained. There is research being done 5,9 that indicates that Cannabidiol has particular benefits as a neuroprotectant. For example, Cannabidiol seems to limit neurologi-cal damage following stroke and trauma or in the treat-ment of neuro-degenerative disorders such as Alzheimer's disease and Parkinson's disease. The current case study, however, is attempting to understand the role of CBD oil in helping apprehensive dental patients obtain their necessary treatment.
