Care should be taken in any molar extraction plan to make sure the upper 8’s are acceptable. If there is a full Class II in the molars the easiest and most elegant plan is to extract the upper 6’s. If the upper 8’s are not available or you have an adult patient that wants shorter treatment time you might best extract the upper 4’s. If the patient is truly retrognathic and there is a significant skeletal discrepancy orthognathic surgery should be considered.
