Carapezza, DMD wrote an article titled: Six Keys to Early Mixed Dentition Class II Correction: A Quantified Approach to Diagnosis and Treatment. Key four is the orthopedic effect. My experience has been that the orthopedic effect may occur spontaneously, may be guided, or a combination of both. Dr. Carapezza writes that unlocking the occlusion allows the mandible to counter rotate and advance sponta-neously 0-3mm. 1 In other cases, guided orthopedic correc-tion with an appliance may be necessary. These appliances may include a bionator (Fig. 1-A), 12 Rick-A-Nator (Fig. 1-B), Twin-Force Bite Corrector (Fig. 1-C), Herbst (Fig. 2-A) 3 and Twin-Blocks (Fig. 2-B) 4 to name a few.
