Carapezza, DMD is a Diplomate of the AOS and has over 25 years of clinical experience in pediatric orthodontics and operates a successful private practice in Wayland, MA. He has degrees from Brandeis University and the University of Medicine and Dentistry of New Jersey. He served as a Teaching Fellow at the Harvard School of Dental Medicine and received a certificate in Pediatric Dentistry from Children’s Hospital. Dr. Carapezza is currently an Associate Clinical Professor at Tufts University, School of Dental Medicine and a contributing editor to the Journal of Clinical Pediatric Dentistry. His lecture experience includes the Senior Certified Instructor for both the American Orthodontic Society (AOS) and International Association for Orthodontics (IAO). Juan Carlos Echeverri, DDS, FAOS is the owner of Echeverri Dental Center in Houston, TX. Fluent in English and Spanish, he has made presentations in Colombia, Venezuela, Spain and the USA in prosthesis, implants and orthodontics including dental education for patients. Echeverri is the creator and director of the dental education outreach program for the schools surrounding his practice. He is a member of the Greater Houston Dental Society, Texas Dental Society, American Orthodontic Society, American Dental Association and Academy of General Dentistry. Raphael Greenfield, DDS, MScD as a honors graduate of Howard University College of Dentistry, Dr. Greenfield received a MS in Orthodontics from Boston University in 1973 and continued as a clinical instructor until 1976. He became a Diplomate of the American Board of Orthodontics in 1984 and exhibited his board cases at the A.A.O. meeting that same year. Lecturing nationally and abroad on his 98.5% nonextraction treatment philosophy, he presented his “Coordinated Arch Development®” philosophy at the 1996, 2003 and 2004 scientific sessions of the American Association of Orthodontists. Publishing extensively, in 1999, Dr. Greenfield authored the first and only textbook entitled Nonextraction Orthodontics: The History, Philosophy and Clinical Application. The originator of five scientific patents and successfully treating more than 10,000 nonextraction patients over 30 years, he also serves as a referee for the American Journal of Orthodontics and Dentofacial Orthopedics. Shirley Gutkowski, RDH, BSDH, FACE is a career dental hygienist and cham- pion of Minimally Invasive dental hygiene. She is the recipient of the World Congress of Minimally Invasive Dentistry Leadership award, and popular author and speaker. Idiculla, DMD graduated at the top of his dental school class at Nova South- eastern University, he attended the University of Pennsylvania for his orthodontic certificate. He then moved to Denver, CO, upon graduating from his residency as chief resident. Idiculla has received numerous awards, including the Dean’s Academic Award, the University of Pennsylvania Department of Orthodontics Research Award, and Top Orthodontist in his area. He is an internationally-recognized lecturer on both clinical and practice management in orthodontics, and most recently, lectured in Brazil and Hawaii to their respective orthodontic societies. He also constantly lectures to orthodon- tic residency programs throughout the United States. Dr. Idiculla serves on the boards of the Colorado Orthodontic Foundation, Kids in Need of Dentistry, and the Colorado Orthodontic Associa- tion. Currently, Dr. Idiculla maintains three thriving orthodontic offices across Colorado. David W. Jackson, DDS, FAGD, IBO graduated from Baylor College of Dentistry in 1978. Dr. Jackson, a member of AOS and AGpO, as well as other professional organi- zations, operates two highly successful practices in Farmersville and Rowlett, Texas and employs over 25 people. He lectures extensively for the American Orthodontic Society and the International Association for Orthodontics. Lyle, RDH, BS, MS received her Bachelor of Science degree in Dental Hygiene and Psychology from the University of Bridgeport and her Master of Science degree from the University of Missouri, Kansas City. She has 18 years clinical experience with an emphasis in periodontal therapy. She has been a full time faculty member at the University of Medicine & Dentistry of New Jersey, Forsyth School for Dental Hygienists and Western Kentucky University. She has contributed to three dental hygiene text- books, published over 30 evidence-based articles and presented numerous continuing education programs nationally and internationally. Deborah is on the editorial board of the Journal of Dental Hygiene and has conducted several clinical studies that have been published in peer-reviewed journals. Currently, she is the Director of Professional & Clinical Affairs for Water Pik, Inc. Clarke Stevens, DDS, MS is an orthodontist in full time practice located in Omaha, Nebraska. He received his dental training at the University of Nebraska. His master's degree in Orthodontics is from the University of North Carolina at Chapel Hill. Dr. Stevens initiated the Dentofacial Clinic at the University of Nebraska and served as Co-director for 15 years.
