However, it’s necessary to do so in order that the patient has a nicely balanced facial skeleton at the end of treatment (Fig. 16-B). Observations and Treatment Plan Considerations The main observation that I make in this case is that this patient has been blessed with an almost ideal facial skeleton. However, the teeth are terribly misaligned. My general default setting in most orthodontic cases is to finish the patient with a full complement of 28 or more teeth. However, this is not always possible. Some-times teeth need to be extracted, particularly in late adolescent and adult orthodontic cases.
